Office Policies

Appointments

In order to best serve you we ask that you arrive 15 minutes prior to your scheduled appointment. In the event that you need to cancel or change an appointment we kindly request that you provide our office with at least 24 hour's notice.

Insurance

Estimated insurance copays are due at the time of service unless prior arrangements have been made with our office manager. In the event that the insurance payment is less or more than expected, the account will be adjusted accordingly .

Minors

Minors (under the age of 18) must be accompanied by a parent or guardian at all times. In the case of divorce or separation, only the parent physically present with the child will be held responsible for the account.
